All prize requests form have been sent out to the contributors that have been received.  The following are the ONLY individuals who still need to submit a prize request form.  (Sorry, I just don't have the time to email everyone individually that their form was received.  When you submitted your form, it directed you to a "Your Submission has been received" page so that should have been sufficient).

The FOLLOWING INDIVIDUALS ONLY need to submit a Prize Request Form before 5PM on TUESDAY, OCTOBER 25, 2011 (Eastern Standard Time).  There is a clock on the home page to tell you what time it is currently.  If your name is listed below - you need to complete the PRIZE REQUEST FORM.  If you have FGP after the prize listed for you - then you need to SELECT (1) FRIEND TO AWARD THE SAME GIFT TO AND HAVE THEM FILL OUT THE FRIEND PRIZE REQUEST FORM.

The following individuals have until 5PM tomorrow to submit their forms.  In the event a form is not received from the winner and/or the friend - a new winner will be chosen.  If a friend is not selected, that additional prize award will be forfeited.
